Confirmation of Understanding

- I have read and understand the CPIT"s contractor induction information
- I understand my responsibilities as a contractor
- I am familiar with CPIT's general emergency, accident, fire safety and evacuation procedures
- I will notify CPIT management of any hazards I will create or discover during the contract
- I will ensure any of my staff or sub contractors are also familiarised with this information prior to commencing work at CPIT
- If I am to be working alone in an isolated area, I will advise Facilities Management or Security prior to commencing work
